Grace Blossom Care is located in the beautiful City of Lakewood, California, in a very quiet neighborhood, surrounded by trees, flowers, and fascinating nature that brings peace of mind and happiness to residents. Our company is owned and operated by a licensed Registered nurse, BSN, MSN, with years of experience working in different levels of Hospital specialties. Our owner rendered her nursing services to home health, Skilled Nursing, in-home hospice care, midwifery, mental/Psychiatric, and geriatric. Our owner decided to bring home the work of her hands, to fulfill her long-term goal and desire of serving and caring for those who once cared for us, by opening a Residential care facility for the elderly (RCFE) to ensure continuous quality care and services in a home-like environment. This Assisted Living facility is licensed by the California department of social services in 2019, to provide care and services to residents from ages 60 and above. Our approved hospice waiver allowed our facility to care for clients with terminal illnesses or injuries. Our team-oriented staff will continue to care and support our clients emotionally and will work together with our clients, families, and any hospice agencies that they may have. This facility is dedicated to providing quality care and services to our residents. We are open 24 hours a day, and 365 days a year. We have trained caregivers who are committed to providing individualized care, and recognition to each resident. We are here to make you happy, create good memories, and work together with you and your loved ones or families.
